Instructions for completing host university
forms:
- Host universities in the United Kingdom and Republic of Ireland require their own application forms to be completed and attached to your EAP application, before they will consider you for admission. To speed up the admission process, please complete the host university application forms noted below for each of your top two choices of UK host university. (East Anglia, Imperial College London, and Leeds are exceptions—you will be notified later in the process if your application will be referred to one of these sites. At that time, you will complete an online application and submit it electronically to that university.) Republic of Ireland students complete a host university form ONLY for the university to which they have applied.
- Scroll down to the appropriate host university and print out the forms, complete each one and submit them with your EAP application. (Ignore any directions about mailing these forms.) The course choices must be made from the host university’s own course listing, own catalog, and/or the EAP Course Finder. Do not choose courses from a University of California catalog.
- Short-term students need to choose courses offered during the appropriate term, so must refer to the host university’s own materials that specify this information for visiting part-time students. The EAP Course Finder does NOT indicate the appropriate courses for short-term students.
- Some host university forms will be from prior years. Updated forms may or may not become available. Do not complete an outdated form too early, but when it is time to complete the form, please complete whatever version appears here.
- Do not staple anything. Documents may be re-arranged and photocopies may be made.
Note: The word “Module” means the same as “course” and “class.” The word “Unit” means the same as “credit.

Tips for submitting supplemental
items:
- Complete the host university forms according to the online sample forms.
- If required, submit two letters of recommendation. You do not need to submit three letters, even if listed as a requirement. The name of a UK host university or England or Scotland should not appear in letters of recommendation for the UK. so they can be submitted to any UK university.
- Transcripts are included in your EAP application, so you do not need to submit extra copies.
- If required, submit a statement of purpose in ONE of the following ways: (a) Print a paragraph in the space provided on the host university form; (b) Write a page-long statement of purpose and do not mention a host university name or England or Scotland, so it can be submitted to any UK university; (c) Write a host university-specific statement of purpose for each host university form
- Do not write the names of your other UK host university choices if the form has a field for this information,. Your EAP application is sent to only one university at a time.
- If applying for Art Studio courses, submit with the EAP application a vinyl sleeve of approximately one dozen slides of your art work.
- Do not submit a host university application fee.
- Inform your campus EAP advisor if your GPA increases during the placement process. The EAP application is not cast in concrete – it is an active, flexible file that can be improved on your behalf. Also inform your campus EAP advisor if your GPA decreases, if you change your major, if you add a special focus to your major, or if anything else occurs which may affect the placement procedure.
- Respond immediately if the host university asks a question or requires another form to be completed.
- Prior to acceptance by a host university, do not communicate directly with them about the status of your application unless they have first contacted you. Channel all communications via your campus EAP office or via UOEAP. Multiple contacts can cause confusion and duplication of effort. After admission, of course, direct communication by the student with the host university is encouraged. The most efficient path is:

- Do not complete the Leeds online Application Form until you have been notified by EAP that your application will be referred to Leeds. At that time, complete the Form and submit it electronically to Leeds. (Check “Exchange” rather than “Fee Payer” in the Present University drop-down menu on the Form.)
- After the electronic submission, a Confirmation will appear. Regardless of the instructions on the Confirmation, the ONLY signature required is your own. Print, sign, and submit the Confirmation to the Universitywide Office of EAP.
- Course Listing For additional instructions on choosing classes, please visit: http://www.leeds.ac.uk/studyabroad/incomers/modules.htm (Please note: Level 1 and Level 2 Studio Art classes within the School of Fine Art are available to exchange students, but Level 3 classes are not. - Do not complete the Edinburgh online Accommodation Form until you have been notified by EAP that your application will be referred to Edinburgh. At that time, complete and submit the Form electronically to Edinburgh (ONLY if you want to reserve on-campus housing).
- Course Listing (Science and Engineering Fall Quarter students must enroll in a minimum of 40 Edinburgh credits (16 UC units) of courses from the College of Science and Engineering. Students wishing to take courses from another College (e.g., Humanities and Social Sciences) must do this as units above the required 40 credits in the College of Science and Engineering.
- Please note: Honors (3rd year) courses in the humanities and social sciences at Edinburgh have very limited space, particularly in the fields of English Literature, History, Politics, Psychology, and Sociology.
